Good Samaritan is also recognized as a Patient Centered Medical Home. A patient-centered medical home (PCMH) is a model of care in which patients work directly with their primary care provider and take an active role in their comprehensive integrated care. The provider works to advocate and arrange appropriate care with other qualified providers and community resources as needed. This model of care is relationship-based which focuses on the whole person. The medical home practice actively supports patients in learning to manage and organize their own care at the level the patient chooses. Recognizing that patients and families are core members of the care team, medical home practices ensure that they are fully informed partners in establishing care plans.
Clinic Hours: Monday, Tuesday, Wednesday & Friday: 8:30 am to 5:00 pm | Thursday: 8:30 am to 8:00 pm
Pharmacy Hours: Monday, Tuesday, Wednesday & Friday: 9:00 am to 5:00 pm | Thursday: 9:00 am to 7:30 pm
Pharmacy is closed for lunch from 1:00 pm to 2:00 pm every day.
This entity receives HRSA Health Center Program grant funding under 42 U.S.C. 254b
and has been deemed a Public Health Service employee for purposes of certain liability protections,
including Federal Tort Claims Act coverage, under 42 U.S.C. 233(g)-(n).
