Patient Rights and Responsibilities:
As a patient, you have the right to:
- Exercise these rights without regard to sex or cultural, economic, educational, or religious background or the source of payment for your care.
- Know the name of the physician who has primary responsibility of coordinating your care and the names and professional relationships of other health care providers who will see you.
- Take part in your health care and treaatment.
- Be treated with respect and dignity in a safe and private setting.
- Respect for your cultural, social, spiritual, and personal values and beliefs.
- Be informed about your illness and treatment, including options for your care, in terms that you can understand.
- Choose and/or change medical providers at Good Samaritan Health and Wellness Center.
- Get another opinion about your illness or treatment.
- Full consideration of privacy concerning your medical care program. Case discussion, consultation, examination, and treatment are confidential and shoud be conducted discreetly.
- Confidential treatment of all communications and records pertaining to your care at Good Samaritan. You will receive a separate "Notice of Privacy Practices" that explains your privacy rights in detail and how we may use and disclose your protected health information.
- Talk with the Chief Medical Officer about questions or problems with your care.
- Know about services available through Good Samaritan health and Wellness Center.
- Ask for special arrangements if you have a disability.
- Know the cost of your care, receive an explanation of your bill, and be informed of ways you may pay for your care.
- Ask for help with a living will or durable power of attorney for healthcare. This includes designating a decision maker if you become incapable of understanding a proposed treatment or become unable to communicate your wishes regarding care.
- Have all patient rights apply to the person who has legal responsibility to make decisions regarding medical care on your behalf.
- Participate actively in decisions regarding your medical care, to the extent permitted by law.
- Refuse to be included in any research program without limiting medical care or treatment.
As a patient, you have the responsibility to:
- Provide complete information concerning past and present illnesses, complaints, medications, and medical history to the best of your ability.
- Take an active role in your health care by asking questions about yur illnesss and treatment plan.
- Inform your medical provider if you become worse or experience any unexpected reactions to your medication or treatment plan.
- Keep your contact information up-to-date.
- Show respect to both caregivers and other patients.
- Assure that the financial obligations of your care are fulfilled.
- Safeguard your written prescriptions, medications, and medical devices so they cannot be used by others.
- Give written permission to obtain your health records from other medical providers when it is necessary to evaluate past medical history.
- Provide Good Samaritan Health and Wellness Center a copy of your living will or durable power of attorney for healthcare matters.
- Cancel or reschedule appointments in a timely manner so that another person may have that time slot. Failure to do so may result in a six-monthsuspension ofservices after three no-shows in a 12-month period
